RESUMEN

The authors reported a newborn female baby, 36 weeks gestational age, 2680 gms who developed respiratory distress at 2 hours after birth. Her chest roentgenograms showed a normal-sized heart, sparse lung markings and bilateral masses involving the posterior 6th to 8th ribs. Thoracotomy with partial removal of the left chest mass was performed when she was 12 hours age. Postoperatively, the baby developed progressive respiratory distress and expired at 29 hours-age. Autopsy revealed bilateral nodular masses arising from the inner side of the posterior aspect of the 6th to 8th ribs, measuring 6 x 5 x 4 cm (right) and 7 x 6 x 4 cm (left). The cut surfaces showed multicystic spaces containing blood. Histologically, many blood filled spaces with walls of fibroblasts, cartilage and bone tissue were noted. The diagnosis was mesenchymal hamartoma of the chest wall. Mesenchymal hamartoma of the chest wall usually arises from the posterior or lateral portions of the rib and usually involves many ribs. Multi focal lesions and bilaterality are rare. The clinical presentation can be asymptomatic, mild or severe respiratory distress. Surgical resection is the treatment of choice.

RESUMEN

This is a report of Ternidens deminutus infection in a 33 year old Thai female who was admitted to the hospital because of abdominal pain and a right lower quadrant mass. Exploratory laparotomy revealed an omental mass with attached terminal ileum. Resection of the mass together with the terminal ileum and the right sided colon was performed. Pathologically, the omental mass was an abscess with an immature male Ternidens deminutus identified. The parasite usually is found in the intestine of primates in Africa and Asia. Human infection occurs when food contaminated with infective filariform larvae is ingested. The larvae molt in the intestinal wall and become adults. They pass eggs in feces. Eggs in contaminated soil hatch and become rhabditiform larvae and then infective filariform larvae.

An investigation as to whether any association of pediatric HL in Thailand was likely to be EBV positive was performed on formalin-fixed paraffin embedded tissue sections using in situ hybridization for EBV encoded RNA (EBER) technique. The analysis was performed on 15 cases. They were 11 male and 4 female cases. The subtypes of HL according to WHO classification were nodular lymphocyte predominance in 1 (6.6%), nodular sclerosis in 4 (26.6%), mixed cellularity in 9 (60%) and lymphocyte depletion in 1 (6.6%). EBV encoded RNA by in situ hybridization was demonstrated in 92.8% of classic HL: 3 of 4 (75%) with nodular sclerosis; 9 of the 9 with mixed cellularity (100%) and 1 of 1 (100%) with lymphocyte depletion. Case of nodular lymphocyte predominance was negative for EBV, CD 15 CD 30 and positive for CD 20. CD 15 and CD 30 were positive in 78.6% and 85% respectively for classic HL. Our results suggest a strong association of EBV with pediatric classic HL (92.3%) particularly the mixed cellularity subtype (100%). The result confirms the male predominance in pediatric HL. Mixed cellularity is the most common subtype of HL in our series (60%).

RESUMEN

Lymphangioma of the spleen is a rare disorder with a clinical manifestation ranging from incidental findings of an abdominal mass to symptomatic abdominal pain. The authors reported a case of splenic lymphangioma with the presenting symptom of frequent urination in a 9-year-old boy. A large firm mass was found at the left upper quadrant, 12 cm in diameter. The diagnosis was confirmed by ultrasonography and computerized tomographic (CT) scan. The symptoms of frequent urination disappeared after operative splenectomy. No evidence of recurrence occurred over a 3-year follow-up.
